We ask that all patients bring your insurance card and photo ID to each appointment. It is also imperative that you bring all of your medications, current prescriptions as well as over-the-counter, with you to every appointment. If you should have any new medical and/or cardiac history, including testing, please bring copies of those records that you have. If you do not have copies you may complete the appropriate form below so that we may obtain those for your medical record at our practice for continuum of care.
We ask that you complete the New Patient Packet to bring to your appointment as this will help to streamline your check in process. You may also fax the completed packet to us prior to your appointment date if you wish.
